It meets, a little 'hidden, at the entrance of Isola del Gran Sasso, just after the bridge over the stream Ruzzo. It was built in 1610, has a gabled roof and a stone facade that has a beautiful portal, also in stone.

Under the large oculus is still readable a writing dedicated to the Mother of God and in several places of the facade and the roof is the symbol of the Order of the Jesuits. The small bell tower is sailing and holds only one bell.

The interior preserves a statue of the Madonna delle Grazie to which the place is named, while behind the altar, on the wall, are still some frescoes, unfortunately marked by time, by neglect and damage caused by earthquakes.
